Intake Forms
Once your appointment request has been approved, you will receive your Welcome Packet. Please review my practice information and complete your forms before your visit! (We hate forms too and we have done everything we can to make sure we don't waste appointment time filling out forms)!
Your First Appointment
Your first visit is a longer appointment. We will take the time to make sure your needs are met, and questions are answered, and we have created a custom plan that is tailored to your goals.
